What are Dermal Fillers?

Dermal filler is a generic term used to describe substances used to replaced volume loss in the face. Despite the name, dermal fillers are often not placed into the dermis, but into deeper structures to achieve the best results.

Dermal fillers can be classified into several types, depending on what they are made of:

  • Permanent
  • Stimulant
  • Temporary

What type of fillers should I have?

At Facial Artistry, Dr Bernard Leung uses temporary fillers. Permanent fillers and stimulant fillers on the whole have a higher side effect profile, such as infection and granuloma formation. These side effects could be difficult to manage. Temporary fillers have a proven safety record. They are extremely safe due to the fact that they are naturally occurring. Technological breakthroughs have made some of the newest fillers very long lasting.

How long does temporary filler last?

The latest generation of dermal fillers is very long lasting indeed, to the extent that they have been called ‘semi-permanent’ fillers.

The longevity of fillers depends on the area being treated. In general, dermal filler injected into a relatively immobile area, such as the cheeks, tend to be longer lasting than the same filler injected into a highly mobile area, such as the lips.

Newer fillers have come on the market with enhanced physical properties, such as variations in the elastic modulus, cohesivity, and viscosity, which allowed different fillers to be deployed at different locations to give even higher degree of lifting, but yet softer, more natural looking result. Some of the new fillers are less hydrophilic and therefore attract less water into the treated area to avoid swelling. This is of particular importance under the eyes.

Certain filler injection techniques have the benefit effect of stimulating new collagen formation, and the clinical effect is therefore much longer lasting than the longevity of the filler itself.

In general, dermal filler treatments by Dr Leung has a clinical effect of between 12 months to well over 2 years.

Are there any side effects?

Swelling, bruising and asymmetry are possible side effects. The dermal fillers that Dr Leung uses are essentially naturally occurring substances which form the soft tissue matrix of the body. They are therefore extremely safe.

The other major advantage of this type of dermal fillers is that the effect is entirely correctable. If for whatever reason you are not completely happy with the results or that there is excessive swelling, the dermal filler can be selectively ’dissolved’ with injection of a naturally occurring enzyme in the body.

Will I benefit from dermal fillers?

Dermal fillers are most commonly used to treat facial volume loss associated with the aging process.

Can it be combined with other treatments?

In most instances, a better aesthetic result is achieved when dermal filler is combined with other treatments, such as anti-wrinkle injections and/or laser treatments. This should be assessed on an individual basis.

Is it painful?

No. People who have a dermal filler treatment are pleasantly surprised at how there is no pain. Dr Leung uses a number of pain relief techniques to minimise any discomfort during the treatment. This includes local anaesthetic cream, nerve blocks and the soft cannula technique of filler placement.
